2022-2023 LinkedUp Youth Partner Application
Thank for considering becoming a LinkedUp Youth Partner. LinkedUp is a program between Teen Health Mississippi and school-based clinics in Mississippi high schools that aims to prepare young people to address their healthcare after high school. LinkedUp Youth Partners will work with THMS staff and clinical partner staff to develop marketing campaigns to make sure their fellow students, especially the senior class, are aware of and interact with LinkedUp. Please complete this form as thoroughly and completely as possible. All applications will be reviewed by Teen Health Mississippi (THMS) staff as well as the LinkedUp Clinical Partner at your school.
